Please note:  You will need to provide a copy of your military discharge papers (DD214).
OR
National Guard/Reserves:  You will need to provide a copy of your 'Activation Orders' where the Authority Line shows Title 10, Subsection 672 or 12301.
If you cannot locate your copy of your DD214 (discharge papers), you will need to contact the National Archives & Records Administration for a replacement copy.  Or you may now access vetrecs.archives.gov to order copies of military records electronically.

All non-electronic requests must use Standard Form 180 or request the information in a letter. Standard Form 180 is available as a PDF file on the NARA website.  The NARA site provides complete information on information required to process your request.
